Situation Ebola en RDC au 28/11/2019

Urgence sanitaire en République démocratique du Congo (RDC) : le duo Ebola et rougeole !

Déclarée le 1er août 2018, l’épidémie à virus Ebola en cours sur le sol de la RDC est toujours active (environ 2200 décès au 28/11/2019 sur 3200 cas confirmés) mais semble ralentir. Les autorités sanitaires du Congo (RDC) et l’Organisation mondiale de la santé (OMS) ont observé ces dernières semaines une réduction sensible des cas de contamination à Ebola dans les provinces du Nord-Kivu et de l’Ituri.

Jusqu’à présent, seul le vaccin expérimental fabriqué par Merck, jugé “très efficace et sûr” par l’OMS, avait été utilisé par la RDC : le vaccin VSV-ZEBOV. Il s’agit d’un vaccin vivant, dont le vecteur est le virus de la stomatite vésiculaire (une maladie qui touche surtout les bovins, porcs et chevaux). Ce vecteur modifié accueille un gène codant pour une protéine de la souche Zaïre du virus Ebola (l’une des cinq souches identifiées du virus, mais la plus létale et responsable de la plupart des épidémies connues).Le vaccin a montré son efficacité et sa bonne tolérance dans une quinzaine d’essais réalisés en Afrique, aux États-Unis et en Europe. Un autre vaccin fabriqué par Janssen, filiale belge de l’américain Johnson & Johnson a été introduit en RDC depuis quelques semaines : Ad26-Zebov. Ce vaccin expérimental contient l’adénovirus 26 exprimant la glycoprotéine du virus Ebola. La tolérance et la réponse immunitaire de ce vaccin semblent satisfaisantes. Mais l’efficacité clinique n’a pas encore pu être démontrée sur le terrain.

Si la situation Ebola semble être sous contrôle, le pays fait actuellement face à une autre épidémie de grande ampleur : plus de 250 000 cas suspects de rougeole associés à plus de 5000 décès ont été signalés en RDC au 17/11/2019. Toutes les provinces sont impactées. Une campagne nationale de vaccination est actuellement en cours et devrait être achevée à la fin de l’année !

Urgence sanitaire sur les Iles Samoa : la rougeole fait (encore) parler d’elle !

Ce n’est pas un cyclone qui menace les îles Samoa actuellement mais une épidémie de rougeole. Ces îles qui abritent environ 200 000 habitants au cœur de l’océan Pacifique sont en état d’urgence pour essayer d’enrayer le phénomène : plus de 4000 personnes infectées et 60 cas de décès déclarés ! L’OMS recommande un taux de vaccination de 95 % pour protéger les populations de la rougeole. Or, aux îles Samoa, un tiers des enfants de moins de 4 ans n’ont pas reçu de vaccin. Et selon les estimations de l’Unicef et de l’OMS, la couverture vaccinale aux îles Samoa est passée de 58 % en 2017 à seulement 31 % en 2018. Afin de lutter, les autorités gouvernementales ont décidés d’adopter des mesures drastiques (fermeture des écoles et lieux administratifs, réquisition des agents gouvernementaux …) pour mettre en place une campagne de vaccination de masse.
